RELEASE, WAIVER, INDEMNIFICATION,
AND HEALTH AFFIRMATION
By signing this Release, Waiver,
Indemnification, And Heath Affirmation below, I intend to be legally bound
hereby, for myself, my minor children, my wards, my heirs, executors,
administrators, successors, and assigns, and in consideration of Camp Twin
Lakes, Inc.’s (“CTL”) permitting me or my child to attend and
participate in activities at CTL’s facility (“Camp Twin Lakes”), I
hereby release and forever discharge CTL and any of its officers, directors,
employees, and agents from and against any and all damages of any kind
whatsoever arising out of any injury, illness, infirmity, disease, or loss
of any kind, personal or property, to me or my child during or related to me
or my child’s attendance at a camp at Camp Twin Lakes.
I understand and certify that me or my child’s participation in
__________________________(“Partnering Organization”) and its
activities at Camp Twin Lakes is completely voluntary and I have
familiarized myself with Partnering Organization’s program and activities at
Camp Twin Lakes in which I/my child/my ward will be participating.
I recognize that certain hazards and dangers are inherent in
Partnering Organization’s activities and programs, and I acknowledge that
CTL cannot ensure or guarantee that the participants, equipment, premises
and/or activities will be free of hazards, accidents and/or injuries.
I further recognize and have instructed me or my child, to the extent
my child will be attending and participating in activities at

RELEASE AND WAIVER OF COPYRIGHT AND OTHER USAGE RIGHTS
By signing this Release And Waiver Of
Copyright And Other Usage Rights below, I intend to be legally bound
thereby, for myself, my minor children, my wards, my heirs, executors,
administrators, successors, and assigns, acknowledging that Camp Twin Lakes,
Inc., (“CTL”) has the right to photograph, videotape, and/or create
other audio-visual materials of me or my child’s participation in activities
of CTL’s facility (collectively, the “Audio-Visual Materials”) and
that CTL has the royalty-free right to use the Audio-Visual Materials of
me/my child/my ward in public relations, marketing and promotional
activities and materials in any medium whatsoever including, but not limited
to, videotapes, pamphlets, and brochures including use in print, radio,
television and the internet. I
further acknowledge that CTL shall have all rights of copyright in and to
such Audio-Visual Materials and may exploit such copyright fully.
I release and waive all rights and interests in and to such
Audio-Visual Materials.

EXHIBIT C
SUMMER CAMPS
The rules and regulations of Camp Twin Lakes are necessary to help ensure a
smooth functioning camp. They
have been established for all participants.
As specified in the camp contract, these rules and regulations should
become part of each camp’s staff manuals, reviewed with your staff members
and are designed to be part of each camp’s rules and regulations, not
separate from them.
1. Golf Carts:
When
assigned, only authorized staff as assigned by the User Camp Director may drive
the golf carts. All authorized drivers must be 18 years of age with a valid
driver’s license, take the CTL drivers orientation and must sign and agree to
all rules governing the use of the golf carts.
No golf carts can be driven into the gym at any time.
2. Meals and Dining
Hall:
Meals are served on time
according to the schedule established by CTL. Please be on time. There is no
late or general take-out service. There is service for children, only, who
cannot leave the medical lodge. The Head Nurse must make arrangements for this
service 30 minutes prior to the start of the meal.
Please do not stand on the
tables and chairs.
Staff
members are responsible for setting the proper example in the dining hall.
Campers and staff are responsible for serving themselves and cleaning up
their tables and around their tables after each meal.
Food fights will not be tolerated.
3. Laundry:
The
laundry is available for emergency (camper) laundry use only.
The laundry is available for NO other use.
The laundry is not for staff member’s clothing.
Arrangements for use must be made with the Partner Organization Camp
Director. Only those authorized
staff may use or be in the laundry.
4. Housing:
It is
the responsibility of all staff and campers to keep their living quarters neat
and clean at all times. Living
quarters will be inspected periodically. Air conditioning is pre-set.
Should changes be necessary – please do not do it yourself – contact the
Partner Organization Camp Director.
Moving furniture around inside the cabin or from cabin to cabin is not
permitted.
NO
FOOD is permitted in the cabins.
5. Telephones:
The Camp Twin Lakes phone
number is 706-557-9070. Messages
can be left on extension #239 or #240.
For calls out of camp – arrangements should be made with the Partner
Organization Camp Director.
6. Pets:
Pets are not
permitted within the campgrounds at any time.
Visitors (parents or guests) should be reminded in writing not to bring
their pets to camp on drop-off or pick-up days.
7. Graffiti:
Graffiti is not permitted
– please help keep Camp Twin Lakes buildings and grounds free of graffiti.
Sidewalk chalk can be used – as long as it is used
on the sidewalks only and not on the
cabin or building walls / posts / or porches.
8. Kitchen and Dining
Hall:
The
kitchen and dining hall will be closed for the day after supper
– it will re-open the next day for breakfast. The dining hall cannot be
used for program activities. No
Partner Organization food or medications can be stored in the kitchen at any
time. The Kitchen is off-limits at all
times to all staff and campers. All special dietary needs should be reviewed
with the Partner Organization Director 30 days prior to the start of the
session.
9. Security:
To provide the proper
security for all campers and staff – (1.) The main camp gate will be closed at
all times and only registered participants and guests will be provided entry to
the campgrounds. (2) All
participants and guests must wear permanent name badges or wrist bands while on
the campgrounds.
10. Parking and speed
limit:
Parking is only permitted in the parking lots. Parking is not permitted behind
the medical lodge. Vehicles are only permitted into camp in the bus circle and
only when dropping off campers or staff -- picking up luggage or supplies. No
vehicles are permitted in the cabin areas at anytime. The camp speed limit is 7
miles per hour.
11. Smoking Prohibited:
As we are a camp for
campers with special medical needs, we discourage everyone from smoking. For
those persons who must smoke, an area has been provided.
This area is located between the kitchen and media center.
Parents and visitors, especially on drop-off and pick-up days, should be
reminded of the smoking policy in writing before they come to camp. SMOKING is
not permitted in any building or outside porch area. Please help keep the Camp
Twin Lakes environment free of smoke and cigarette butts.
12. Office:
The Camp Twin Lakes office
building is open daily from 8:30am to 9:00pm.
The office is for official business only and is not intended to be used
as a meeting location by Partner Organization campers or staff.
13. Valuables:
Valuables should be checked with the Partner Organization Camp Director.
Camp Twin Lakes is not responsible for loss or damage to personal
property.
14. Library / Creative
Arts Building Media Center:
No
food and/or drinks
are permitted in the carpeted sections of the library, media center and creative
arts building. The use of paint, colored chalk, glue, rice, glitter, or other
pro/ ducts that could damage or stick in the carpet is also not permitted.
15. Camp Furnishings:
Please do NOT move
furniture within the buildings or from building to building or room-to-room
without permission from the Camp Twin Lakes Director.
Benches have been provided in the recreation building for your
convenience. NO CHAIRS are
permitted to be moved from any one building to another or used outside.
16. Personal Sports
Equipment:
The use of all personal
sports equipment (skate boards, bikes, skates, roller blades, etc) are permitted
but only under the supervision of the User Camp Director and is the liability of
the Partner Organization. All individuals using such equipment must sign a
waiver for such use and must agree to use the equipment in a safe and controlled
manner with the proper safety equipment. Please see the Partner Organization
Camp Director for this waiver form. The use of ‘firearms’, and ‘archery
equipment’ are not permitted on the campgrounds at any time.
17. Program Areas:
The program areas of Camp
Twin Lakes can only be used when supervised.
Arrangements for the use of these areas are made by the User Camp
Director and must be reviewed in advance with the CTL Program Director. Program
areas are for campers use only, no scheduled ‘staff’ sessions can be arranged.
Only the assigned counselors or activity leaders are permitted in the
program areas during camper activity periods. The following specific program
areas can only be used when supervised by a Camp Twin Lakes staff member.
These areas are the: horseback riding area / and horses, ropes and GI
courses, boats, swimming pool, bikes, ceramics studio, archery range, staff
lounge and photo studio.
18. Drugs:
Possession and use of
illegal drugs (marijuana, narcotics, acids, etc.) and misuse of prescription or
OTC medications is strictly prohibited at Camp Twin Lakes.
Camp Twin Lakes has the right to search any belongings if any prohibited
items are suspected. Camp Twin
Lakes will notify the proper authorities if illegal substances are
found in camp.
19. Alcohol:
In accordance with Georgia
law, no one under the legal drinking age may be in possession of or drink
liquor, beer, or other intoxicating substances at any time.
Drinking of alcoholic beverages while in direct care of campers is
strictly prohibited. Each group
using Camp Twin Lakes must inform the Camp Director/Manager of its alcohol
policy, and receive prior approval before alcohol is permitted on property.
20. Weapons:
Weapons of any kind are
prohibited on site by campers, visitors and staff.
Potential weapons in a person’s possession should be turned in
immediately. Firearms, knives, and
other potential weapons intended for program uses should be locked when not in
use and only used in the presence of trained and/or certified staff.
Inform Camp Twin Lakes’ personnel when these items are being brought in
for programmatic use along with written policies and procedures.
21. Thermostats:
Damaged thermostat covers
or thermostats that have been tampered with will result in a fine up to $250 to
the user group. It is the goal of
Camp Twin Lakes to reduce costs and conserve energy where possible.
